Ulquiorra Cifer died against the Hollowed version of Ichigo Kurosaki during the Arrancar Arc. Ulquiorra faced the wrath of the ultimate hollow, the beast that made up half of Ichigo and ravaged the lands of the hollows. He quickly disappeared into dust when Orihime tried to touch him but here we see the rebirth of the Fourth Espada.
